Leaders at the top are utterly lost; and worse, they are completely fine with being utterly lost because they have no oversight. Therefore, they have no need to be accountable.
Because this is a perpetual cycle, nothing will ever change since they hold both the power to make and push uninformed decisions and the knowledge of their lack of leadership and people management expertise. This combination has resulted in a command and control environment, no matter how pretty a picture they try to paint it to be.
