Turnover galore paired with the "in-group" dynamic of a family-run company results in a workforce that has either been at the company for 10+ years, or less than three years, with little in between. Management has no idea how to respond to change and will make wholesale changes with little advance notice, rhyme or reason to the decisions. Chronic staff shortages result in products going out late, which perpetuates the issues of rash decisions being made. They actively choose not to follow many of the systems and protocols that have been put into place to solve some of the issues.
