We're excited to share that we recently celebrated the grand opening of our new CenterWell Senior Primary Care® center next to Walmart in Gladstone, Missouri. This is the first of 23 new locations opening near Walmart stores across four states this year. Our Gladstone center has been welcoming patients since December 2024, but the grand opening was a special moment to connect with the community. We hosted tours, live music, bingo, food, and more—bringing people together to experience what makes CenterWell unique. As part of Humana’s Primary Care Organization, CenterWell and our sister brand, Conviva, serve nearly 390,000 seniors across more than 340 centers nationwide. Our mission is to provide affordable, personalized care—especially in underserved communities. We designed this center with seniors in mind, from easy parking to dedicated entrances. Being next to Walmart makes it even more convenient for our patients. Our value-based care model helps patients stay healthier with more frequent visits and preventive care. In fact, seniors in models like ours had significantly fewer hospital admissions last year compared to those in Original Medicare. Our team includes board-certified clinicians, behavioral health specialists, pharmacists, and more—all trained to care for seniors. We also offer community activity spaces open to everyone. Humana Launches CenterWell Home Health One year after launching the CenterWell brand to represent Humana’s payer-agnostic healthcare service offerings, Humana Inc. has started to transition Kindred at Home’s home health division to the CenterWell Home Health brand.
Humana Adds 14 States to Phase 2 of CenterWell Home Health Launch Humana Inc. has announced the addition of 14 states to the ongoing initiative to rebrand the home health division of Kindred at Home (KAH) as CenterWell Home Healthopens new window. The announcement of Phase 2 of the initiative brings to 21 the number of states where KAH home health services have transitioned – or started transitioning – to CenterWell Home Health.
