Since our inception, we have experienced significant growth, expanding our reach and impact within the community. Our commitment to providing comprehensive follow-up healthcare and support services has allowed us to serve a diverse range of customers, including displaced individuals referred by hospitals, as well as homeless individuals in need of specialized care.

We provide a wide range of services, including post-hospital medical care, medication assistance, behavioral health and addiction prevention services, homeless case management, housing transition assistance, transportation to/from appointments, crisis management services, and more.
