Our company mission is to provide comprehensive physician-guided care to patients who are home-bound or prefer home care through a coordinated interface with specialists and healthcare providers.
BridgeCare provides home visiting physicians, on-call physicians, wellness assessments for Medicare Advantage plans, telehealth and diagnostics at home throughout Illinois. The team may serve seniors in assisted living, independent living, retirement communities, senior apartments and private homes. Our physician services are available 24 hours a day, 7 days a week and take great attention to tailor the patient's plan of care to produce the best outcome.
Our providers coordinate care with the patient's Home Health Agency, PCP and specialists involved with their health care. Our team partners with home visiting nurses, therapists, dietitians, Podiatrists, homemakers, caregivers, social workers and medical supply
companies to ensure continuity of care.
