There can be some breakdown in interdepartmental communications which causes some issues. Very fast paced environment that is constantly changing. The things you know one day are completely irrelevant the next. It is sometimes very difficult to maintain a high quality of output when the understanding you have is incorrect.
Too much emphasis on making shareholders happy and not doing what is right for the long-term success of the company.
Top heavy management (too many chiefs and not enough indians)
Poor business model that relies heavily on the whims of other companies/partners in order to keep them in business
Agile development methodology with no serious QA and a lack of understanding/committment to doing what it takes to develop a better product
Virtually all development was outsourced to India with poor communication in between
