Unrealistic expectations from management without putting any emphasis on training. Promotes culture of fear and blame where employees are afraid to ask questions- I've had a peer procrastinate for hours on a number of tasks for fear of asking the supervisor for help. Out of date IT systems which further slow down efficiency.
