Bad management - Lack of leadership behaviours. No mentoring / guidance. Immature / unable to make sensible decisions. Culture - Staff do not have Agile mindset. Put politics as priority rather than meeting projects goals. Staff need to be educated on ethics / integrity on how to treat colleagues with respect which undermine collaborations / agility / inclusion.
