CEO is controlling beyond belief. No one is empowered to make decisions, everything must funnel through him, regardless of how obvious the solution is. We were all yelled at for missed goals and threatened with repercussions, but when the next review rolled around nothing had changed and the same people who failed to deliver previously had failed to deliver again, so we all got yelled at again.
Upper management has ZERO trust in the abilities of their rank-and-file employees, and if you somehow manage to prove you're right or come up with a good idea on solving an issue the middle manager will claim it as their own and present it to the CEO for implementation. Deny that if you like, I watched it happen.
