A 30-year-old male sustained a crush injury from a trailer, resulting in multiple comminuted fractures to his metacarpals and proximal phalanges. A manual laborer, the patient wanted a minimally invasive approach that achieved stable fixation for early range of motion (ROM) so that he could return to work quickly. Dr. Sisco-Wise decided to use intramedullary (IM) fixation with the ExsoMed™ INnate™ IM Threaded Nail for the metacarpal fractures and the ExsoMed InFrame™ IM Threaded Micro Nail for the phalangeal fractures because both implants were minimally invasive and offered rigid fixation and early mobilization unlike other surgical approaches. The total surgery time for the entire hand was approximately one hour. At the two-week postop, the patient experienced no pain and could perform early active ROM. By four weeks postop, the patient was cleared to return to work and daily activities without any restrictions. In this case by Nicholas DiMassa, DPM, the patient was a 44-year-old female who slipped and fell down the stairs. The patient sustained an unstable, long spiral oblique fracture and rupture of the tibiofibular syndesmosis on her right ankle.
