Description About us

Care Design NY is the largest Care Coordination Organization/Health Home (CCO/HH) in New York State. We provide person-centered comprehensive care management services for more than 27,000 individuals with intellectual and/or developmental disabilities (I/DD) , so they may lead a quality life. We utilize a person-centered and holistic service planning approach to coordinate care and deliver integrated supports and services with individual, family and provider input.
