Upward Change Health Services provides quality, community-based services to individuals with developmental disabilities, mental health disparities, and substance abuse victims. Individuals are supported in accordance with their needs in school, work, home, and the community. Services are person centered which means they are designed by the individual, the individual’s family, and others who know the individual well. Services are rendered in the environments in which the individual desires to live, work, recreate, or pursue educational goals, and are designed to support each to pursue his or her own self-directed life goals.
