HUMAN INTEGRATION Adobe Care & Wellness is the only technology-driven health care company of its kind that looks at the whole person and addresses their needs at all levels. While IT may lead the way, our integrated team of nurses, nurse practitioners, social workers, support coordinators, dieticians, care navigators, and transition specialists work together to improve the quality of life for the individuals they serve.
TRANSITIONAL CARE To help ensure a smooth recovery and keep the patient from further decline or the need to be readmitted, contact is initiated after 24 hours at time of discharge. Adobe Care & Wellness completes an in-home assessment to reconcile home safety issues and establish follow-up care as needed.
PREVENTATIVE CARE We work directly with insurance providers on improving their STARS ratings and closing HEDIS measures consistently delivering quality care to their assigned members. The Adobe team provides healthcare services such as: Screenings Check-ups Vaccines Management of chronic and acute illnesses Educational events and campaigns Comprehensive wellness assessments Telemonitoring and Telemedicine check-ups
INTEGRATED CASE MANAGEMENT We take a data-driven integrated approach to case management. The member is the hub that is supported by our team of care coordinators, social workers, transition specialists, dieticians, nurse practitioners, nurses, and care navigators. We conduct in-person home safety assessments to uncover any gaps or disparities that may impede a member’s health. Working collaboratively, the team monitors and addresses the members’ health and wellness based on their needs.
Description Adobe Care & Wellness began as an idea for a healthcare solution dedicated to improving lives. When Founder Jayme Ambrose was studying for her doctorate, she developed a holistic and integrative healthcare model for a class project. At her professor’s urging, Ambrose took her thesis project from concept to reality.
In 2012, The Beech Group agreed to pilot her model, if Ambrose agreed to accept a position as VP of Clinical Services. In 2016, she signed her first insurance company contract and initiated her holistic approach to case management for Medicare, Medicaid, and commercial members.
In 2017, when The Beech Group decided to sell the company, Ambrose was presented with an opportunity to purchase her division. She agreed and opened the doors to Adobe Care & Wellness with the mission: To positively impact the lives we touch.
The company grew from 12 employees and one office to more than 250 employees and four offices in its first three years. Today, Adobe Care and Wellness employs an in-house team of techies developing software solutions that allow the company to track vital information. Utilizing data to identify disparities and gaps helps to proactively address the needs of its members. The company now provides healthcare solutions and services to more 200,000 lives across multiple states.
