Autumn Leaves is a family owned and operated assisted living community solely dedicated to the care and well-being of those living with Alzheimer’s and other forms of dementia. Our communities are designed from the ground up specifically to address the unique needs of those with memory loss.
Each of our assisted living communities is staffed with community managers and nurses who are trained to specifically work with those living with dementia and Alzheimer’s. We only care for individuals and families dealing with the issues of memory loss.

Our team understands more than the disease, they understand your loved one. Our staff gets to know each one of our dementia care residents on a personal basis. We learn their names, their history, their favorite foods, how they like their coffee, and their personal talents. This is memory care that is as individual as our residents.
