We are a network of Nurse Practitioners, Physician Assistants, and Doctors who see patients in private residences, assisted living communities, skilled nursing facilities, clinics, and via telemedicine throughout South Carolina and Georgia. We understand that a person’s home is where they reside, whether that is a Skilled Facility, Assisted Living, Independent Living, or private residence. We provide every service you would expect from a traditional Doctor’s office visit directly to the patient in their home. Patients can choose to see a medical professional in person or via Telehealth. At Your Health, we currently serve SC and GA providing House Calls and Primary Care services, including personalized care plans, medication management, testing/labs, specialist referrals, follow-up appointments, remote patient monitoring, and more.
