Description Community Health Center Network (CHCN) is committed to excellent, affordable care for underserved communities in the East Bay. We are a non-profit Medi-Cal managed care organization, providing business administrative support for community health centers to focus on what matters most -- patients.
Community Health Center Network was formed by our member health centers in 1996 to support health centers in a rapidly expanding managed care environment. Its sister organization, Alameda Health Consortium, was founded by community health centers in 1974 to support advocacy, health insurance enrollment and service coordination for uninsured and low-income communities.
CHCN is a designated Innovation Hub, an initiative of Center for Care Innovations with support from the California Healthcare Foundation, and one of four national Technology for Healthy Communities pilots.
