Lack of any defined process and all the many other problems that brings from planning, to customer expectations, to career growth, to safety, and to budgetting. A lack of direction and meaning in most tasks, day to day. An old boy network of one employer per career middle management that has no interest in doing anything differently and will actively defend the status quo no matter what. Senior tech and business management with no real breadth of experience, just a lot of doing it "the Bell way"
