Sorry to hear you had this experience. I realize this was written a long time ago, but what you state is completely inaccurate and bears no resemblance to SBDS today. No QA? Really? We have a highly automated and rigorous QA process, which includes individual unit tests at the developer level, up through multiple levels throughout the build process, culminating in an automated series of integration testing. No code is released without going through this very structured process. This discipline is the case for all types of release, including patches. Everything top to bottom is done within a Structured Agile Framework, with clear deliverables and process to avoid just the panic that you are suggesting.
