locally: high employee turnover leading to a shortage of employees available to deliver product/service unhappy customers and employees poor communication unrealistic expectations micro-management company-wide: too many meetings, conference calls, processes and forms keeping you from getting your job done compensation outdated/homegrown productivity tools such as email and enterprise systems - very clunky and inefficient
