Almost Family, Inc. (NASDAQ:AFAM) was founded in 1976 and has over 340 locations in 28 states. Almost Family and its subsidiaries operate four segments:
1. Visiting Services (VS), which provides skilled nursing and physical, occupational and speech therapy services primarily to Medicare beneficiaries
2. Personal Care (PC), which provides custodial and personal care services
3. Hospital Partnership Division (HPD), which provides skilled nursing and physical, occupational, and speech therapy services, as well as hospice services in partnership with hospital systems
4. Health Care Innovations (HCI), our division that drives technology and innovations throughout the entire organization; includes Imperium ACO Management company
