Vision care is VSP which really limits who is "in-network." Go out of network and only get $50 reimbursed for exam. Annual lens allotment is puny compared to other employer plans. Dental is pretty much the same as everywhere else--Delta Dental. Generally, expect to pay 10% out of pocket on the HSA (after $1500 deductible which Amazon contributes $500). On the PPO, you'll pay the 10% as well after $300 (no Amazon contribution), but your prescriptions are on a more traditional co-pay scale. If you have prescriptions of any significant cost, you'll probably get killed on the HSA."
