About Community Health Center of Fort Dodge Founded in 2006, Community Health Center of Fort Dodge was created through the collaboration of local churches, physicians, agencies, and businesses who recognized a pressing need for accessible healthcare in Fort Dodge, Iowa. Since then, we’ve grown into a multi-site organization with locations in Fort Dodge, Mason City, Dayton, Eagle Grove, Clarion, and Spencer. We are a federally qualified health center (FQHC) providing comprehensive medical, dental, behavioral health, pharmacy, and optometry services to individuals and families—regardless of insurance status or ability to pay. With a focus on inclusivity, bilingual support (including Spanish-speaking staff), and community-based care, we serve patients of all ages across rural north-central Iowa. Our integrated care model ensures coordinated, holistic care under one roof. Services include: - Preventive and primary medical care - Dental services - Behavioral health counseling - 24-hour on-call coverage - Social work services for community resource referrals - Financial advocacy and assistance with sliding fee scale eligibility and enrollment History of Expansion: 2006 – Opened in Fort Dodge as an FQHC 2014 – Began providing medical care in Dayton 2018 – Opened Mason City location with medical, dental, and behavioral health 2023 – Expanded to Eagle Grove (medical and dental) and added behavioral health later that year 2024 – Opened our Clarion location 2025 – Opened our newest location in Spencer We’re proud to serve and grow with our communities.
